Patient Registration Form

New patients, NOT being seen for concussion care, need to download this form and submit it to our office using one of several methods:

  • They can be filled out electronically, saved as a new copy and emailed to team@snowsportandspine.com

  • You can print out the blank form, fill it out, then scan the completed documents and email them to team@snowsportandspine.com

  • You can bring printed, completed paper copies with you to the appointment

  • If none of the above methods work for you we can provide forms at our office when you arrive. Please be sure to allow 15 extra minutes before your appointment to allow time to complete the forms in person.

Help Using Our Forms

Our forms are .pdf files that can be filled out electronically. You can open them using a few methods:

  • You can use Adobe Acrobat Reader. If you do not have it and would like to install it, please go to their download website.

  • You can open them in your web browser (Google, Firefox, MS Edge).

 

In order to use these applications you will need to have associated opening .pdf files with them, if you haven't done so already. Follow these instructions to automatically have all .pdf files open in the viewer of your choice:

  • Right-click on the PDF file after you download it

  • Scroll down to Open With and select Choose Another App

  • Click the box next to "Always use this app to open .pdf files"

  • Then click on the browser you want to use or Adobe Acrobat Reader

  • Click on OK

COVID Policy

COVID Screening

Our office is still requiring masking to keep our communities safe. If you do not have one we will supply one for you.

 

You will be asked a few questions, if answer yes to any, we will be unable to treat you that day:

  • Have you tested positive for COVID in the last 14-days?

  • Have you been exposed to anyone, that you know of, that was positive in the last 14-days?

  • Are you having any COVID symptons today?

You will be asked to either use our hand sanitizer in reception or liquid hand soap at the sink. You then be invited to take a seat in the waiting area until your therapist is ready for you.
