YOUR FIRST VISIT
What to expect
When you arrive at Snow Sport and Spine, our front desk staff will assist you during check-in to complete a brief COVID screening, ensure we have any documents we need, and then seat you in our waiting area where your physical therapist will greet you. They will devote the time necessary to listen to your concerns, review your medical history, and ask about your goals and expectations for your care. Your therapist will ask questions to help understand your problem, and as important, about your lifestyle and particular situation. From there, they will complete a thorough evaluation, then share the findings, and together, create a plan that works for you. Before leaving the visit, a final discussion will consist of the length and frequency of visits that will be needed to achieve the goals set out by you and your therapist. This will include strategies, prognosis, and home exercise instructions. You are encouraged to participate in every aspect of the rehabilitation process, as your contribution is vital to a successful outcome.
Check-in Process
Cancellation Policy
Please arrive at least 10 minutes before your appointment, to allow adequate time to get checked in. You will receive an email that contains a registration form, which needs to be filled out electronically and emailed back, or printed and brought in to your first appointment. A consent form and medical release will be signed in person. Any additional medical documents that you can provide will be scanned and returned to you. You will be introduced to your therapist and directed to a treatment room.
We require a 24-hour notice of all cancelled appointments. A full visit fee is charged for all scheduled visits that are missed, or canceled with less than 24-hours notice. We understand that you cannot always control your circumstances, and in the event of an illness or a positive COVID test, we will waive the fee. If you have an emergency, or encounter inclement weather, please contact us as soon as possible. Appointment reminders are sent out 48-hours before your scheduled appointment.
Payments
Our rate is $160/hour. Depending on your presenting goals and issues, the initial evaluation could be 60 to 90 minutes and will be determined ahead of time. We accept cash, credit card, Health Savings Account (HSA) cards, or checks. We are an out-of-network provider and do not accept insurance. We will be happy to supply you with documentation to submit to your insurance company for out-of-network reimbursement or to apply towards a deductible, click below to learn more.

COVID Screening
Our office is still requiring masking to keep our communities safe. If you do not have one we will supply one for you.
You will be asked a few questions, if answer yes to any, we will be unable to treat you that day:

Have you tested positive for COVID in the last 14-days?

Have you been exposed to anyone, that you know of, that was positive in the last 14-days?

Are you having any COVID symptons today?
You will be asked to either use our hand sanitizer in reception or liquid hand soap at the sink. You then be invited to take a seat in the waiting area until your therapist is ready for you.
